there was a write up about it..... really good and that's where i found it
 
yes, the 45RFE has 2 second gears in it and one is not used when its the 4 speed set up... the TCM is separate from the PCM and no programming needed, i swapped it into the truck and i now have a 5 speed auto trans
Thats not where the extra gear comes from.. the 45RFE does already make use of both 2nd gears. "2" is used during normal shifting, while "2 prime" is used during hard acceleration and kickdown.

The extra gear is a 2nd overdrive. It does NOT kick in when OD is off or if the speed does not go above 48mph. This is a major concern if the truck is driven mostly in the city becuase it will actually hurt your mileage a bit.

Because it is a 3 planetary transmission with no bands, the 45/545RFE are actually capable of producing 10 forward gears and 2 reverse gears. some are unused because they are overlapping or not wide enough gap.. but thats how you are able to "program in" the extra gear by doing the TCM swap.
